1) Primary Forms of Photo Identification (One Required): Must include a clear photograph

• Matricula Consular (Mexico), NEXUS (Canada)

2) Secondary Forms of Identification (One Required): required to verify the validity of the address

• Current Lease, Mortgage, or Deed of Trust
• Voter or Vehicle Registration card
• Home or Vehicle Insurance Policy
• Form I-94, Arrival and Departure Record
